Secondary Caries: What It Is and How We Treat It

Caries is one of the most common dental diseases. But did you know a tooth can decay even after a filling? This is called secondary caries.

What is secondary caries?

It’s the re-decay of tooth structure under or near an existing filling, crown, or restoration. It occurs when bacteria enter microgaps between the tooth and the filling material.

Main causes:

  • Poor seal between tooth and filling
  • Inadequate prior treatment
  • Wear of the restoration over time
  • Poor oral hygiene
  • Hidden decay missed during earlier check-ups

Symptoms:

  • Pain or sensitivity when biting
  • Discoloration around the filling
  • Bad breath
  • Roughness felt with the tongue
  • Gaps between the filling and tooth

Note: Secondary caries may be asymptomatic.

Treatment includes

  1. Removal of the old filling
  2. Thorough cleaning of the cavity
  3. Root canal treatment (if needed)
  4. New filling or crown

Prevention tips

  • Visit your dentist regularly (2x per year)
  • Monitor old fillings
  • Act immediately on symptoms
  • Maintain hygiene: brushing, flossing, rinsing
  • Use remineralizing products if teeth are sensitive
